Allied Health Professionals

Chronic Disease Management Nurse

Chronic disease is a health condition that can be managed or improved but not cured (e.g. asthma, diabetes, COPD). Complex care patients are those with 2 or more chronic diseases at the same time, which requires coordinated treatment. The Mosaic PCN recently added several new programs within this initiative, including a Mosaic Community Clinic to enhance the current chronic disease programs available through member physician's offices.

As part of the services that Mosaic PCN offers, CDM nurses play a large role in managing chronic disease issues within Mosaic physician's offices. Together with the physicians, CDM nurses assist with case management, referrals to appropriate services, and provision of disease management according to clinical practice guidelines.

The purpose and goal of this service is to help patients with chronic disease or complex care needs to manage their disease(s) and to live as independently as possible. The focus of this program has continued to be on poorly controlled diabetes, pre-diabetes, hypertension and dyslipidemia. Other diseases that Mosaic PCN also addresses are Chronic Obstructive Pulmonary Disease (COPD) and asthma.

Dietitian

MPCN Dietitian goal is to provide nutritional expertise to patients with chronic diseases. They assess, plans, implements, evaluate and consult with patients to promote health project and strategies. They help patients to improve their diet and improve their health by developing and implementing dietary care plan by assessing nutritional needs, determining resources and coordinating dietary program.

Pharmacist

MPCN Pharmacist goal is to provide answers on your medication questions, review prescriptions, instruct and teach you how to take proper medication and educate you on the risks, benefits and side effects of medications.

First appointment will be consultation by assisting you on how will you take proper medication. Next will be a detailed care plan for a patient who includes implementation and monitoring strategies. Follow up appointments is needed for some medication or drug recommendations.

Social Worker

MPCN Social workers provide services on a one-to-one basis with the patient. They are helping individuals, families, groups and communities to enhance their individual and well-being. They help people develop their own skills and their own ability to use their own resources to resolve problems. They are concerned with the patient with personal problems and social issues such as poverty, unemployment and domestic violence. They are helping patients in their financial problems by educating them on budgeting and debt management. They provide community resources like employment counselling, disability support, legal services and guiding them for available housing options.
